Every claim here is graded by what the evidence is, not by how good the mushroom sounds.

This is the most commercially loaded corner of the subject and the one where a reader is least likely to be told the truth. Nothing on this page is medical advice, nothing here treats or prevents any disease, and none of it belongs near a decision about actual medical treatment.

The scale, strongest first

    A regulator somewhere has assessed a specific preparation for a specific use and licensed it. This is a different thing from a supplement containing the same species.

    Randomised controlled trials in people exist and point somewhere, but they are typically small, short and often industry-funded. Promising is not proven.

  3. 03Human trials, weak or mixed

    Trials exist and disagree, or are too small and too poorly designed to conclude anything from.

  4. 04Animal and cell studies only

    Effects seen in mice or in a dish. This is the stage at which most supplement marketing quietly starts, and the majority of compounds that look good here do nothing in people.

    Long recorded use, which is a genuine reason to investigate something and not a reason to believe it works.

Before you buy anything in this category

Fruiting body, or mycelium on grain

A large share of mushroom supplements sold in the United States are not made from mushrooms. They are made from mycelium, the fungal thread network, grown on a bed of sterilised grain, and then the whole thing, grain included, is dried and powdered together. The label may say mycelial biomass, or full spectrum, or simply name the species.

This matters because the resulting powder can be substantially starch from the grain. Independent analyses have repeatedly found high starch and low beta-glucan content in such products, and beta-glucan is the compound class nearly all of the research is about.

The tell is on the label and it takes ten seconds. A product that states BETA-GLUCAN content as a percentage is measuring the thing that matters. A product that states polysaccharides is quoting a number that includes starch, and starch is not what anyone is buying it for. If the ingredients say mycelium, mycelial biomass, or list a grain, you are buying grain.

None of this decides whether these mushrooms do anything. It decides whether the jar in your hand contains the material the studies were done on, which is a prior question and a much easier one to answer.

Hericium erinaceus

Hericenones in the fruiting body and erinacines in the mycelium, both of which stimulate nerve growth factor in laboratory work.

Cognition and memory in older adults

Human trials, promising

A Japanese randomised trial in adults with mild cognitive impairment found improvement on a cognitive scale during supplementation, which faded after it stopped. It involved 30 people. Later trials have been similarly small, and it has not been replicated at a scale that would settle anything.

Mood and anxiety

Human trials, weak or mixed

A handful of very small studies, mostly in specific populations, with inconsistent results.

Nerve regeneration

Animal and cell studies only

The nerve growth factor effect is real in a dish and in rodents. Whether the relevant compounds cross the human blood-brain barrier in a capsule is not established.

The honest summary

The best case among the popular functional mushrooms and still a long way from proven. The honest position is that a small, unreplicated trial in 30 people is a reason to keep studying it, not a reason for the claims printed on the packaging.

Who should be careful

Generally well tolerated. Rare allergic and respiratory reactions have been reported.

What is actually in the jar

Insist on fruiting body. Lion's mane is one of the species most commonly sold as mycelium-on-grain, where the erinacines are in the mycelium but so is a great deal of grain starch. A supplement listing beta-glucan content, rather than vague polysaccharides, is telling you something real.

Best evidence: Approved as a drug

Trametes versicolor

Polysaccharide-K, known as PSK or krestin, and polysaccharide peptide, PSP. Both are protein-bound beta-glucans.

As an adjunct to chemotherapy in specific cancers

Approved as a drug

PSK has been an approved adjunct cancer therapy in Japan since 1977 and is among the most-used such agents there, with meta-analyses in gastric and colorectal cancer showing a survival benefit. That is an approved pharmaceutical preparation used alongside conventional treatment under medical supervision. It is not a capsule bought online, and it is not a treatment for anything on its own.

General immune support in healthy people

Some measurable changes in immune cell counts in small studies. Whether that translates into a healthy person being less ill is not shown.

The honest summary

The strongest evidence base of any mushroom here, and the widest gap between what the evidence supports and what is sold. A regulated drug used alongside chemotherapy in Japanese hospitals is a completely different product from a supermarket capsule, and the marketing relies on you not noticing.

Who should be careful

Anyone in cancer treatment should raise this with their oncology team rather than adding it independently. Interactions with treatment are a real question and not one to guess at.

What is actually in the jar

Look for extract standardised to beta-glucan content. Turkey tail is thin and leathery, so a whole-mushroom powder is largely indigestible fibre; the active fractions are hot-water extracted for a reason.

Reishi

Best evidence: Human trials, weak or mixed

Ganoderma lucidum

Triterpenes, which give the intense bitterness, and beta-glucans.

Fatigue and quality of life in illness

Human trials, weak or mixed

Several small trials with modest results. Cochrane has reviewed reishi for cancer treatment and concluded the evidence does not support it as a first-line treatment, while noting possible adjunct effects worth further study.

Sleep and calm

Animal and cell studies only

Sedative effects in rodents. Human evidence is essentially absent.

Immune modulation

Human trials, weak or mixed

Changes in immune markers in small studies, with no clear clinical outcome attached.

The honest summary

Two thousand years of recorded use, a great deal of laboratory work, and human evidence that stays stubbornly thin. It is the mushroom where the gap between reputation and demonstration is widest.

Who should be careful

May increase bleeding risk and is a genuine concern alongside anticoagulants. Also lowers blood pressure in some people. Both are worth raising with a doctor rather than discovering.

What is actually in the jar

Dual extraction matters here more than for any other species. The triterpenes are alcohol-soluble and the beta-glucans are water-soluble, so a water-only extract is missing half of what the species is taken for.

Chaga

Best evidence: Animal and cell studies only

Inonotus obliquus

Betulinic acid and related triterpenes drawn from the birch itself, plus melanin and polysaccharides.

Antioxidant activity

Animal and cell studies only

Very high antioxidant capacity in laboratory assays. High antioxidant readings in a test tube have repeatedly failed to translate into human health outcomes across many foods, and this is no exception.

Immune and anti-inflammatory effects

Animal and cell studies only

Rodent and cell work only. Human trials are essentially absent.

The honest summary

The weakest human evidence of the group, combined with the clearest documented harm, which is an unusual and unflattering combination.

Who should be careful

Avoid entirely with any history of kidney stones or kidney disease. It also affects blood sugar and may increase bleeding risk.

What is actually in the jar

Two problems worth knowing before buying at all. Chaga is very high in oxalates, and there are published cases of kidney damage in people taking it heavily over long periods. And it is being harvested faster than it forms: a conk takes years to grow and taking it can kill the birch.

Cordyceps

Cordyceps militaris, Ophiocordyceps sinensis

Cordycepin and adenosine analogues.

Exercise performance and oxygen uptake

Human trials, weak or mixed

Small trials, several of them in older adults, showing modest improvements in oxygen uptake measures. Trials in trained athletes have generally been less impressive, and most studies used a specific commercial preparation rather than the species in general.

Fatigue

A little supportive evidence, all of it small.

The honest summary

The most consistently over-claimed of the group. The effects seen are real but small and mostly in unfit or older participants, which is a very different claim from the performance marketing built on top of them.

Who should be careful

May affect blood sugar and bleeding. Caution alongside diabetes medication and anticoagulants.

What is actually in the jar

The species question here is not pedantry, it is the whole purchase. The traditional material is Ophiocordyceps sinensis, harvested wild from caterpillars in the Himalaya, and it is one of the most expensive biological products on earth. Almost everything sold as cordyceps is cultivated Cordyceps militaris, or a mycelium-on-grain product. Those are three different things at three wildly different prices, and a cheap jar labelled cordyceps is never the first.

Shiitake

Lentinula edodes

Lentinan, a beta-glucan, plus eritadenine.

As an adjunct in gastric cancer treatment

Approved as a drug

Lentinan is licensed in Japan as an injectable adjunct to chemotherapy in gastric cancer. Injected, prescribed, and unrelated to eating shiitake or taking a capsule.

Cholesterol

Human trials, weak or mixed

Eritadenine lowers cholesterol convincingly in rodents. The human evidence is old, thin and inconsistent.

The honest summary

The most sensible entry on this page, because you can simply cook and eat it. Shiitake is genuinely nutritious food with a licensed pharmaceutical derived from one of its compounds, and those two facts are not the same fact.

Who should be careful

Raw or undercooked shiitake causes a striking whip-mark rash in a small proportion of people, called shiitake dermatitis. Cooking it properly destroys the compound responsible.

What is actually in the jar

Buy it as food, fresh or dried, and grow it on a log if you want a supply. There is no good argument for a shiitake capsule when the mushroom is in every supermarket.

Beta-glucans, particularly the fraction marketed as D-fraction.

Immune markers

A small trial in breast cancer patients found measurable immune changes, in both directions depending on dose, with no clinical outcome demonstrated.

Blood sugar

Consistent effects in rodents. Human data is minimal.

The honest summary

Like shiitake, this is best treated as excellent food rather than as medicine. The evidence for the supplement is thin; the argument for roasting it is overwhelming.

Who should be careful

May lower blood sugar, which is worth knowing alongside diabetes medication.

What is actually in the jar

Buy it to eat. It is cultivated widely, sold fresh in many supermarkets, and among the best mushrooms there is in a hot oven.
